Effect of class 2 elastics in treatment of orthodontic class two division one malocclusion: a pilot study

OBJECTIVE: To study the dental & skeletal effects of class 2 elastics in relation to reduction of overjet and overbite in the treatment of mild to moderate class two division one malocclusion using pre- and post treatment ceph al ometri c values. MATERIALS & METHODS: A pilot study on 15 patients (male:female ratio 8:7) selected at the Jinnah Medical & Dental College, Orthodontic department undergoing treatment with fixed orthodontic braces. The ages ranged from 11 to 15 years (mean age 13.4 years). Inter-maxillary class 2 elastics were attached from upper canine bracket hooks to lower 1st permanent molar bands bilaterally. All patients were provided with 1/4th heavy (6.4 mm) 6.5 oz intra-oral elastics. Patients were instructed to wear the elastics 24 hours except meal-times. Elastics were worn fulltime for an average 9 months to one year. No untoward patient complaint was noted except for occasional de-banding of the lower molar bands due to vertical pressure of the class 2 elastics in few patients. Mean calculated total treatment duration was 22 months. RESULTS: Skeletal, dental and soft tissue parameter changes were recorded.The lower labial segment showed proclination while the upper incisors retroclined during treatment. Extrusion of the lower 1st permanent molars was noted with mild increased lower facial height. Both naso-labial and labio-mental angle showed post-treatment changes. CONCLUSION: Class 2 elastics showed dental-skeletal and soft tissue changes in class 2 division one correction. However, our next study with greater patient size will be done to investigate the long-term effects of class 2 correction with elastics.
