Aspergillus brain abscesses in a child with acute lymphoblastic leukemia

A 2-year old boy was diagnosed with acute lymphoblastic leukaemia (L1,PAS-ve,SBB-ve)and was started on ALL Regimen A according to the MRC-ALL 97/01 protocol. He presented with a consolidation in the right lower lobe after having had respiratory symptoms, fever and no neurological signs. A lung biopsy showed Aspergillus Fumigatus. A CT Brain was performed which showed multiple ring enhancing lesions with surrounding edema consistent with multiple cerebral abscesses.It was decided to give Ambisome (liposomal amphotericin B) for 3 months and additional Voriconazole. Follow up CT Scans showed dramatic improvement and a decrease in the size of all the lesions. Both chemotherapy and surgery have been used in combination to treat Aspergillus brain abscesses. At present Ambisome, azole antifungals, GCSF, GMCSF(granulocyte macrophage colony stimulating factor), MCSF(macrophage colony stimulating factor) and interferon-gamma are available treatments.
