Case report and discussion of a Pregnant woman with cholera

A 23 year old, eight weeks pregnant house wife was referred from a local clinic after six days of treatment where she presented with vomiting and loose motions that started with 3-4 episodes of vomiting and 3-4 semisolid stools per day. This gradually progressed to profuse watery diarrhea of 20-25 bowel movements per day resembling `rice water`. She had decreased urine out put for the last three days but did not complain of abdominal pain or fever. Due to protracted diarrhea it was decided to use injection Octreotide, a somatostatin analog, 50µg subcutaneously 8 hourly (Octreotide is safe in pregnancy). Within 2-3 hours of the first dose of Octreotide, her stool consistency changed to semi-solid. Frequency also decreased gradually and after 48 hours the loose stools stopped. Octreotide was given for 3 days. On day two stool culture was reported positive with Vibrio cholera (serotype not known). The patient was discharged two days after stopping Octreotide, and remained well.
