BACKGROUND: Off-pump coronary artery bypass surgery is currently used as an alternative to conventional "on-pump" surgery. Advantages of avoiding extra corporeal circulation are numerous. The aim of this study was to review our initial experience with off-pump coronary artery bypass surgery at a local institution. METHOD & RESULTS: Prospective consecutive case controlled study at a tertiary care teaching hospital. Seventy seven consecutive off-pump coronary artery bypass surgery cases were compared with 74 similar cases performed by conventional "on-pump" technique. There were 10 deaths altogether, 3 in the former and 7 in the later group. CONCLUSION: It is concluded that off-pump coronary artery bypass surgery is safe and has some advantage over conventional "on-pump" technique. It is particularly recommended in patients under going emergency bypass procedure for acute coronary syndrome, and for those who had no lime to discontinue antiplatelet medications before surgery.
