Transvesical Prostatectomy: Still a good option

Objective: To evaluate perioperative and postoperative complications of suprapubic transvesical prostatectomy. Design: A department based longitudinal descriptive study. Place and Duration of Study: Department of Urology, Bolan Medical College and Sandeman Provincial Teaching Hospital, Quetta (Pakistan) over a period of 6 years (January 19.95 to December 2000). Subjects and Methods: A total of 480 patients of benign prostatic hyperplasia (BPH) undergoing transvesical prostatectomy were studied for perioperative and postoperative complications for 6 months. Results: The age range of the patients was from 44-98 years and the average age was 64.6 years. In perioperative complications, 1.9% patients required blood transfusion intraoperatively and 2% patients postoperatively. Total mortality rate was 0.6%. Early postoperative complications like wound infection, urinary tract infection and epididymo-orchitis were present in 5.8%, 4.4% and 1.2% patients respectively. Transient incontinence was present on catheter removal in 3.1 % patients, which improved spontaneously with time. One patient (0.2%) developed permanent incontinence. No, patient required repeat prostatectomy. Conclusion: Transvesical prostatectomy having a very low incidence of major complications, requiring no special equipment and expensive disposable, is an excellent procedure for areas of low health care resources.
