Urinary retention due to Hematocolpometra

We present two cases of imperforate hymen with hematocolpometra who presented to us as cases of acute urinary retention in adolescent females. Surgical drainage of the collection was associated with complete resolution of symptoms in both patients. Case Report: A 14-year-old girl was brought to our department with retention of urine for 18 hours. She gave history of lower abdominal fullness and pain for four months, which was especially more severe once a month. Physical examination revealed a cooperative teenager with distended and tender urinary bladder. She was catheterized, 1800-cc of urine was drained, but the mass was still palpable. During catheterization it was noticed that she had normal pubic hair distribution with normal labia and imperforate hymen which was bulging. On digital rectal examination a mass was felt on anterior rectal wall. She was unmarried and had not yet achieved menarche. Her vital signs were normal, blood pressure was 120/70 mmHg, pulse 90/min, respiration 24/min and temperature 98.6o F. An ultrasound of abdomen revealed an enlarged uterus, full of thick fluid, dilated cervix and vagina.
