Clinicopathological features and bone marrow histological pattern in chronic lymphocytic leukaemia

We analyzed 126 previously untreated patients of Chronic Lymphocytic Leukaemia (CLL) with respect to their presenting clinical and laboratory features. The mean age of the patients was 62.5 years (range 36 - 88 years). There were 98 finales and 28 females with M/F ratio of 3.5:1. The bone marrow trephine pattern was observed and correlated with the clinical stage of their disease. In accordance with Rai's staging the patients were distributed as follows: O, 2%; I 16%; II, 20%; III, 38%; IV, 24%; and according to Binet staging system the distribution was A, 13%, B, 31% C, 56%. The most frequently found physical finding was lymph node enlargement (95%) which caused patients to seek medical advice. Spleen was enlarged in 82% and hepatomegaly was seen in 38% of the cases. Anaemia was present in 39%a and thrombocytopenia was present in 30% of the cases. The mean lymphocyte count was 108x10911 (Range 15 - 410X109/I). There were only four patients with more than 5% prolymphocytes in blood and bone marrow aspirate. Bone marrow infiltration with >80% lymphocytes was present in most of the cases. The diffuse pattern of infiltration was seen in 62% of the cases and correlated well with the clinical stage of their disease. It is concluded that CLL in our patients has no major distinctive presenting features except for an increased proportion of males and more incidence of anaemia and thrombocytopenia.
