Analysis of peptic ulcer peforation cases at CMC hospital, Larkana

A retrospective study carried out from January 1997 to December 1999 at Surgical Unit II, Chandka Medical College Hospital, Larkana showed 42 admissions due to Peptic Ulcer Perforation. Out of these 34 (80.95%) cases were males with a mean age of 37.53 years and 8 (19.05%) females with a mean age of 38.77 years, the male to female ratio was 4.25:1. The eldest patient in our study was 68 years and the youngest 17 years. The majority of patients i.e. 71.48% came within 48 hours. The mean duration of perforation was 32 hours. In 13 (30.95%) cases a single perforation was found in the lesser curvature of the stomach, while in 29 (69.04%) cases there was a single perforation in the first part of the duodenum on its anterior surface. Simple closure with viable omental patch was performed in all cases except one, which was treated conservatively. Patients with longer duration of perforation had increased morbidity due to wound and respirator tract infections. The over all mortality was 4.76%, which was due to delay in reporting to hospital, old age, leakage from the repaired site and other associated medical Illnesses.
