A prospective study of typhoid perforation in children at Quetta

Typhoid ileal perforation is very common in developing countries and carries a high mortality rate. The purpose of this prospective study was to review the outcome in children who underwent surgery for typhoid perforation from March 2000 to April 2003. Attention was paid to pre-operative and post-operative factors. There were 50 boys and 15 girls in this study. The sex ratio was 4:1 in favor of boys, with the age range of 3-14years. The mean age at presentation was 10 years. Early presentation, vigorous resuscitation, single perforation and early operation were associated with good prognosis. Late presentation (>10 days), multiple perforations favored fistula formation and less chances of recovery. Common postoperative complications were wound infection, wound dehiscence, residual intra-abdominal abscesses and entero-cutaneous fistulae. The overall mortality rate was 9.2%, though it was 20% in those who presented late.
