Total Knee Arthroplasty: A clinical trial with Posterior Cruciate Retention in patients with Rheumatoid Arthritis and Osteoarthritis

This study was conducted to evaluate the functional results regarding pain, range of motion, stability and patient`s satisfaction in pts after total knee replacement with posterior cruciate ligament retention in pts with osteoarthritis and rheumatoid arthritis This was a prospective study of 30 patients operated in the department of Orthopedics unit I Pakistan Institute of Medical Sciences, Islamabad from January 2001 to September 2003, because of failed conservative measures in patients of OA and RA of knee. Posterior cruciate ligament retaining total knee arthroplastv was carried out in all patients (30) and same surgeon performed all operations. There were nine (30%) male patients and twenty-one females (70%) with average age of 60.7 years (45-64). Twenty-Our (80%) patients were suffering, from osteoarthritis and six (20%) had rheumatoid arthritis. The average knee score improved from 23. 70 points preoperatively to 76. 7 postoperatively and average, functional score improved, from 6.0 to 56.4. Two pts developed wound infection post operatively one settled with antibiotics, other underwent multiple debridments and finally prosthesis had to he removed. At maximum follow up of twenty months no implant loosening was found in any patient. Using rationale of knee society clinical rating system, knee score was found excellent in 6 pts (20%), good in 18 patients (60%), and fair in 3 pt (10%), poor in 3 patients (l0%). Conclusion: It may he concluded that outcome after posterior cruciate retaining total knee arthroplasty with PFC Sigma knee systems (J&J orthopedics) gave excellent pain relief and satisfactorv, functional results.
