A situation analysis of health management system of the province of Punjab was conducted with a view to determine the factors that affected the performance of the public health sector. Complex and multi-tiered organizational structures, centralized planning, centralization of authority, undefined goals, unclear management processes, decision-making without adequate information, lack of job description and career structure, inappropriate and inadequate training of the staff, over-staffing and inappropriate distribution of trained personnel, lack of performance-based incentives, subjective performance appraisal, ineffective monitoring and supervision, lack of financial powers, problems in provision of pharmaceuticals, supplies and equipment, insufficient IEC services and materials, political interference, staff absenteeism, security of female staff, weak referral system, non-coordination of services, ineffective checks and balances at all levels, minimal community involvement and low level of community awareness were the main issue hampering the optimal working of health management system.
