Planned Vaginal Delivery Verses Elective Lower Segment Ceasarean Section for Breech Presentation at Term

Background: Breech presentation at term accounts for 2-4% of all deliveries. Perinatal mortality and morbidity in term breech infants is 5 times more than those born as cephalic presentation. To reduce this, there has been a considerable support in favor of caesarian section (CS) during the past years. Methods: This retrospective study was conducted in Women and Children Hospital Abbottabad to compare maternal and neonatal morbidity and neonatal mortality in planned vaginal delivery versus elective caesarian section for breech presentation at term. Results: 4675 patients were delivered from 1st January, 1997 to 31st Dec., 1998. Out of which 133 consecutive singleton breech births occurring at more than 36 weeks of gestation were selected for the study representing 2.8% of all deliveries. It was found that the incidence of maternal complications was higher in the elective LSCS group (37.5%) than in the vaginal delivery group (23.3%). No difference was noted in corrected neonatal morbidity between the planned vaginal delivery group and elective LSCS group.
