Blood cyclosporin levels in bone marrow transplant recipients

A study was carried out to evaluate the current dosage protocol of oral cyclosporin in bone marrow transplant recipients by monitoring drug levels using trough mode of sampling. It was a cross-sectional study carried out at Armed Forces Institute of Pathology in collaboration with Armed Forces Bone Marrow Transplant Centre Rawalpindi from April 2002 to June 2003. Thirty cases of bone marrow transplant recipients taking oral cyclosporin preparations (Neoral) were selected. Clinical information e.g. name, age, sex diagnosis, date of bone marrow transplant, etc were recorded. Blood samples were drawn just before intake of drug.(trough level). Four samples were collected from each patient, 1st sample in 2nd week, 2nd sample in 3rd week and one sample per month for two months. Blood cyclosprin levels, were determined by monoclonal fluorescence polarization immunoassay on TDx autoanalyzer (Abbott). Out of total 120 determinations 82.5% were within therapeutic range of 100-400 ng/ml. The mean levels at 2nd week, 3rd week, 2nd month and 3rd month were 206,238,194 and 169 ug/mI respectively. It is concluded that the present dosage protocol of oral cyclosporin in the bone marrow transplant recipients studied was found to be appropriate to maintain trough level in the therapeutic drug range of 100-400 ng/ml.
