The association of clinical T-category with tumor markers in prostatic carcinoma

Prostatic carcinoma is one of the five most common tumours of male in northern Pakistan. A simple descriptive prospective study of 50 cases of prostatic carcinoma diagnosed from July 1992 to June 1993 in the Histopathology and Chemical Departments of Armed Forces Institute of Pathology and Urology department of Combined Military Hospital, Rawalpindi was carried out. This study was planned to see the association of clinical T-category done by digital rectal examination (DRE) in cases of prostatic carcinoma with serum prostatic acid phosphatase (PAP) and prostate specific antigen (PSA) levels. The patients were seen between 50-102 years of age with an average age of 70.9 years. Twenty-eight (56%) of the patients were seen in TO and T1 categories. Twenty nine (58%) patients were having serum PSA levels between 10.0 ng/ml and 50.0 ng/ml, whereas thirteen (26%) cases showed PSA assays > 50 ng/ml. The sensitivity of PSA test was 84% in these cases. Thirty-five (70%) patients had serum PAP assays more than 3.7 U/L (70% sensitivity). The levels of serum PSA did not reveal direct and significant correlation with the advancing T-categories probably some tumours in T2 and T3 category stages were unable to secrete enough PSA to raise its levels according to clinical T- category. The PAP levels were found to increase with the advancing T-categories. The serum PAP showed better association with the advancing T categories as compared to serum PSA.
