Evaluation of patient with acute undifferentiated fever

Acutely febrile patients present a common but demanding problem in differential diagnosis. In most cases a careful history and physical examination reveals diagnostic clues, so laboratory studies can be used selectively. The evaluation of persistent fever can be more demanding. The initial office evaluation should help determine the proper pace of diagnostic testing and need for therapeutic intervention. If the illness is slowly progressive and patient is clinically stable one may proceed with diagnostic workup in a deliberate manner on an ambulatory basis, utilizing serial clinical observation and time as key diagnostic tools.
