melasma its generate on skin increase the pigmentation level. the treatment for melasma you can use hydroquinone cream. it is best cream for melasm treatment.

Some important aspects about the treatment must be kept in mind.

* It is a slow process which may take a few to several months, so be patient and stubborn.
* Cure depends upon the depth of pigmentation. If only upper layer is involved (epidermal melasma), it will have better prognosis than dermal melasma, in which deeper layer is also affected.
* Melasma associated with pregnancy usually fades out within months after delivery, although it can reappear during subsequent pregnancies.
* Similarly, if oral pills are the cause of melasma their discontinuation results in self improvement.

Topical medicines include

BLEACHING AGENTS
The most common bleaching agent is Hydroquinone. It is available in 2% and 4% preparations. These should only be used under medical supervision and can cause severe reactions on certain skin types. Lightening agents like kojic acid, derived from fungal or mushroom plants, are often combined with other ingredients like azelaic acid, glycolic acids, lactic acid, retinol, ascorbic acid, and botanical lighteners such as licorice extract and bearberry extract.

SUN BLOCKS
Exposure to sun light is the major culprit; hence, it should be avoided at any cost. A good sun block with SPF 30 (minimum) should be used, but in tropical regions SPF 60 or even 90 is recommended.

TRETINOINS
Tretinoins (Retin A) can be used alone or in combination with bleaching agents. Some preparation contain a combination of hydroquinone, tretinoins and mild steroids, (Tri- melasin).

OTHER MEASURES
In case of failure of above treatments or in very deep hyper pigmentations other measures like chemical peeling, exfoliation, light freezing of skin with nitrogen or microdermabrasions are done. Sometimes lasers are used for removing hyperpigmentary patch.

Melasma is a common acquired hypermelanosis that occurs exclusively in sun-exposed areas; it is exacerbated by sun exposure, pregnancy, oral contraceptives, and certain antiepilepsy drugs.

TREATMENT

Successful treatment of melasma involves the triad of sun blocks, bleach, and time. Without daily use of sunscreens, treatment will fail. Sunscreens should be opaque, broad-spectrum formulations with an SPF of at least 30. Bleaching preparations include 2% or 4% hydroquinone-containing creams or gels. Combination products containing sunscreens and hydroquinone are now available and are very effective and convenient for the patient. These are applied once daily. Simultaneous use of topical tretinoin gel (0.025%) is usually required as well.
