A good case and an excellent management. We here give training to our junior doctors about ACLS/BLS, but do not KITS or simulators available. Our own hospital is not willing to help. Once a Pakistani doctor sent us the kits but they got trapped in the custom, and we failed to get them out.

Training ACLS/BLS to paramedic staff and medical students should be made compulsory.

cardiac arrest in a 17 yr old

the pt was visiting his aunt who was hospitalized. In the hallway he had a syncopal episode. Nurses came to the scene and attached a monitor / defibrillator.
Pt was found to have VT/VF. He was shocked a total of 6 times and was given IV amiodarone. He was intubated and transfered to ICU on an amiodaroe drip. Initially he was sedated with IV Propofol. As his rythm stayed stable his sedation was eased. He was changed form assist control to pressure support on mechanical ventilation and was extubated in 6 hrs. Workup for hypertrophic septum including echo and metablic disorders was negative. Next morning he underwent electrophysiological studies and was found to have an abberent pathway (wolf parkinson white syndrome). Radiofrequency ablation was performed and he was discharged. Its now 2 mon since this happened. On followup appointment he is as active as any 17yr old kid.

Lessons: Be aggressive. Know your CPR and ACLS.
