first need to confirm the diagnosis is correct.make sure the person has nothing life threatening for example cutaneous T cell lymphoma( although unlikely as seen in more older people).
If it is eczema then a therapeutic trial with a good dose oral steroid will clear it up.The next step would be a good history particularly environmental including contact( occupational).
usually these patients end up on low dose oral steroids along with antihistamines to provide relief of itching.unfortunately long term oral steroid can have deleterious effcts. Therefore other options are available but have to be used with caution and by physicians having experience.
These include phototherapy(PUVA ), cyclosporin,etc.

Treatment of Eczema
21 year old male with very severe eczema, effecting arms, legs, trunk and face. Extremely itchy. Effecting quality of life. Has tried a few topical steroids creams such as dermovate, betnovate etc. with limited improvement.
He wants something to either cure the problem or significantly improve his quality of life. What should one consider to treat him?
