According to one of the article written by an Indian Author, he said that if ALT is raised 5 times along with bilirubin, then following measures should be taken:

1. If TB is acute (TBM, Miliary)

Substitute 2nd line safe antituberculous drugs and as soon as ALT comes to normal, then replace the drugs one by one (least toxic first).

2. If TB is not acute

Stop all ATT and monitor ALT. When it comes to normal, add safe drugs first till the full chemotherapy.

Are there any other guidelines beside this ?

It should be treated as Drug induced hepatitis. ATT is not relevant. What is the Liver Function test specially ALT, GGT. Is there Jaundice?

ALT should come down after stopping the drug. Systemic steroids will also help (with the risk of reactivation of TB). Has patient completed the course of ATT therapy.

These all factors to be considered before coming to a conclusion regarding the best therapeutic option.
