I would recommend against spending 25$ on the article referred above by 'chameed'. It is about development of the USSQ and doesn't contain the USSQ itself (which is what I think you are looking for). Your best bet is to contact authors of the papers who have either developed USSQ or validated it. For example you can e-mail Dr. Monga of U of MN at endourol@yahoo.com or you can e-mail Dr. Joshi of Bristol Urology at hrishibjoshi@ukonline.co.uk. I am hopeful contacting them will be helpful.

This creature called 'drrathore' has posted some links and email addressess for full text articles. Did you try those? LOL, may be they are all phony, hahahahahaha.
The article is in Journal of Urology, Mar. 2003, pages 1060-1064 and the cost is $25.

I need assisstance in finding about the "Ureteral Stent Symptom Questionnaire (USSQ)"
I was able to come up with the research articles based on USSQ, but unfortunately couldn't find any material on the Questionnaire itself (regarding its contents, components etc). I would appreciate some help in this regard.
