The "master key" for success in IMM and all other exams is "good training" in the wards. in exam , you have to examine the same patients with same findings which come across in routine clinical practice. for theory, obviously one has to study the text book and short books

If you are aiming for a sub-speciality after IM exam then prepare extra hard for it, because if you fail consistently then your subspeciality training starts and your head is still stuck with IM (which is of course a different subject area).
The papers are a lot easier now that the short essay questions have been dropped altogether and replaced by SBQs alone (a lot more people will pass hopefully).
TOACS is the main bottleneck in this exam. As explained earlier by bvhospital you have to pass some quality time in the wards with real patients, plus for radiology visit CPSP at Lahore or Karachi and take a look at CXRs, CTs, etc which are lying over there (most of us didn't knew that radiology aids existed at CPSP).
For medicine ECGs, CXRs, CTscan of head, chest, abdomen, pictoral questions (from Forbe's Atlas) come all the time in TOACS.
Practice history taking and counselling beforehand as well. Know all the relevant procedures (you may be asked at one station) and all the instruments (one station is dedicated to it)
Attend some mock toacs exams at your institute (no mocks at Hyderabad as always) as that helps a lot too.
Prepare hard on your first attempt as there is too much pressure on the second and third attempts.
