PakMediNet Discussion Forum : Medicine : High Grade Fever with Rash and Disorientation
A middle aged person 45/M, presented with 1 week history of High grade Fever without rigors/chils, and for which he took Tab. Cotrimaxozole for it. Ater 2 days of treatment, he develped rash on his body. Intially it was on abdomen, later on spreaded to whole body. It remained for next 1 week. 1 day prior to admission in hospital, he developed disorientation and drowsiness. No history of any fits or unconciousness.
On examination he found to have pulse of 120 /min with irregular rhythm. Rash was nonblanchable maculopapular. His neck was stiff with positive Kernig. He was drowsy wtih sluggish response, pupils b/l reactive, and downgowing plantars. Fundoscopy showed mild blurring of optic discs.
CSF was done which has cells of 10, with 100% lymphcytes, Proteins / Glucose normal.
Differential diagnosis of Encephalitis/Meningitis, Measles, Cerebral Malaria was made.
He was put on Iv Ceftiaxone for 6 days, IV artemeter for 6 days. No response was seen. Fever remained as high grade.
Blood cultures reported growth of Klabsella.
What are the possibilities ?

Viral Meningitis

Viral meningitis is the right thinking. This patient recovered dramatically after within 3-4 days. Dramatic improvement favors the diagnosis of viral meningitis.
