unmarried female with pain both lumbar regions

a lady aged 30 years has complaints of pain over both lumbar areas for last 3 years.with dysurea and haematuria.she complains of intermittent fever but never found on examination.her repeated ultrasounds show no lesion in kidneys or any pathology on ultrasound.
various antibiotic trials prooved inefective


i have advised her manoux to go for any involvement of tuberculosis in kidneys.Am i going on right direction.

I would like to check her Urea and electrolyte levels, and check her creatinine clearance to assess renal function. Also please do an ESR or CRP. Check any history of weight loss. Dont forget to send Urine sample for microbiology culture.

Thinking about Tuberculosis in our setting would be a wise decision. Mantoux is non specific and usually gives less clues. Get her Renal function tests also as mentioned by Memon. Ultrasound abdomen should also be done. TB Elisa is a good test to detect TB in a better way.

Dysurea spells wrong. The correct spelling are dysuria.

hello memon and others,for that case i got help of my freind who is fcps in medicine ,and assistant professor in teaching hospital,he simply labelled CONVERSION ATTACKS and opted for abntispasmodic and urinary alkalizer.or in other words pscyco somatic origin
