Carcinoma of larynx: analysis of topographical distribution and causative factors.

OBJECTIVE: To analyse topographical distribution and different causative factors contributing to laryngeal cancer. DESIGN: Prospective study SETTING: This study was conducted at department of ENT, Head and Neck surgery unit-II Allied Hospital (Punjab Medical College) Faisalabad. STUDY PERIOD: November 2003 to October 2005. PATIENTS: Fifty (50) cases presenting with carcinoma of larynx were analysed regarding topography and different causative / predisposing factors. RESULTS: Topographically, cancer of larynx involved the supraglottic region in 12 patients (24%). 21 patients (42%) had glottic carcinoma while only one patient (2%) had subglottic laryngeal carcinoma. The transglottic growth was noted in 16 patients (32%). Out of 50 cases, 46 patients (92%) were males and only 4 patients (8%) were females. The age of presentation of carcinoma larynx in our study ranged from 34 years to 78 years. The highest incidence i.e., 42 patients (84%) of laryngeal cancer, occurred in the 5th, 6th and 7th decades of lif e. Smokers were 38 patients (76%), while nonsmokers were 12 (24%). Only 3 patients (6%) gave the history of alcohol intake while rest of the 47 patients (94%) denied intake of alcohol. In our study we also noted that 12 patients (24%) had association of laryngeal cancer with tumours in other family members. The analysis of socio-economic status of the patients showed that majority of the patients i.e 36 patients (72%) belonged to lower class. 12 patients (24%) belonged to middle class while only 2 patients (4%) were that of upper class. CONCLUSION: Majority of the patients of laryngeal cancer have glottic growth in our setup. Male to female ratio is 11.5:1. Majority of the patients belong to 5th, 6th and 7th decade of life with mean age 60 years. Smoking is the most important and leading predisposing factor for carcinoma of larynx.
