Prognostication of acute severe pancreatitis: which one of the Ranson`s and the Apache-II scoring systems can serve as a better prognostic tool in our set-up ?.

Objective: To evaluate the accuracy of prognostic parameters employed in the Ranson`s score with the APACHE II scoring system against actual disease outcome and to determine the applicability of each scoring system in terms of clinical viability, finance, time and reproducibility to our setup. Study Design: Descriptive comparative study. Place and Duration: Department of General Surgery, Pakistan Institute of Medical Sciences (PIMS), Islamabad from October 2006 to December 2006. Materials and Methods: 25 patients aged 20yrs-70yrs of both sexes purposively sampled. Data collection was done using a proforma and data were analysed using SPSS version 14.0 for Windows software. Results: 56% (n = 14) of the patients recovered fully. 20% (n = 5) developed complications and 24% (n = 6) died. In 64% of cases studied, the APACHE II scoring system gave a predicted mortality value that was closer to the actual disease outcome than the Ranson`s score. Conclusion: The APACHE II score proved to be the better prognostic marker in terms of clinical viability for acute severe pancreatitis in our set up. The Ranson`s score is less time consuming and more reproducible.
