Concomitant urothelial carcinoma and prostatic adenocarcinoma.

An 86 years old gentleman presented with a complaint of gross hematuria for the past one year. X-ray and ultrasound of bladder showed echogenic foci in the left lateral and posterior wall of the urinary bladder. Biopsy was performed and showed a papillary urothelial carcinoma, low grade, according to the WHO/ISUP consensus classification. Invasion into the lamina propria was seen. Prostatic urethra was also biopsied at that time and did not show any evidence of tumor. Later, the patient again presented with gross hematuria, and cystoscopy showed gross disease. Radical cystoprostatectomy was performed. The specimen was sent for histopathology. Representative sectioning was performed. Sections from prostate showed adenocarcinoma (Gleason`s grade major 4, minor 3, Gleason`s sum 7), with positivity for Prostate Specific Antigen (PSA) on immunohistochemistry. The prostatic carcinoma was seen to infiltrate into the wall of the urinary bladder (confirmed by immunohistochemical positivity for PSA).
