Our experience of managing posterior uretheral stricture by Cold Knife Optical Internal Urethrotomy at teaching institute.

Objective: To evaluate the efficacy and outcome of cold knife optical internal urethrotomy in the management of posttraumatic posterior urethral stricture. Design: An observational prospective study. Place & duration of study: The study was conducted in the Department of Urology , Services Hospital/ Post Graduate Medical Institute, Lahore from Jan 1998 to April 2000. Patients and methods: Eighty male patients of post traumatic posterior urethral stricture revealed by their urethrogram were included in the study. After complete evaluation, patients were managed by internal urethrotomy. Follow-up was done at 1,3, 6,9 and 12 months. Results: Age range of the patients was 17 to 61 years with the mean age of 33.5 years. Road traffic accident was the main cause found in 60 (75%) patients. After facing the traumatic urethral injury, there was history of attempted foley catheterization in 19 (22.5%), bouginage in 5 (6%) and intervention by rail road technique in 6 (7.5%) of the patients. All patients were with suprapubic catheter when presented to us. The length of stricture varied from 0.3 to 2.0 cm (mean 1.4 cm). Clinical response of internal urethrotomy ,at the end of the study was, “good” in 60.%, “fair” in 25.71% and “poor” in 14.28%.. Conclusion: Optical internal urethrotomy is safe and effective treatment for short post traumatic posterior urethral stricture.
