Frequency of depressive illness with type 1 (insulin dependent) diabetes mellitus in children.

Objective: To assess the frequency of depressive illness with type-1 (insulin dependent) diabetes mellitus in children reported at endocrine clinic, NICH, Karachi. Study Design: Case series study: Place and duration of study: This study conducted during the period of six months from 23rd June, 2006 to 22nd December, 2006 at Endocrine OPD, National Institute of Child Health, (NICH) Karachi. Patients and methods: Diagnosed cases of diabetes mellitus who reported in endocrine OPD were enrolled in this study if they fulfilled the inclusion criteria having age of 7 years to 17 years of either sex, type-1 (insulin dependent) diabetes mellitus and more than one year duration of illness. Exclusion criteria include mentally and physically handicapped child, previous history of psychiatric illness and less than one year duration of diabetes mellitus. Socio-demographic data were collected in semi structured proforma. The instruments used for diagnosing depressive illness included Children Depressive Inventory (CDI), while severity of illness was assessed by Children’s Depression Rating Scale (CDRS). Statistical analysis was done through SPSS computer software (version 10.0). Mean was calculated for all quantitative variables like age and duration of diabetes mellitus. Results: One hundred diagnosed cases of type-I diabetes mellitus were enrolled. Fifty five (55%) were males and forty five (45%) were females with ratio 1.2:1. Mean age of children with standard deviation (S.D) was 11.75 + 2.16 years of age distribution. Out of 100 cases of diabetes mellitus, 13 were diagnosed as depressive disorder. Seven (53.84%) were males and six (46.15%) were females. Regarding severity of illness, it was observed that six (53.84%) cases were moderately depressed, five (38.46%) were mildly depressed and one (7.69%) case was severely depressed. Conclusion: High frequency of depressive symptoms in children with type-1 diabetes mellitus were reported which suggest that early diagnosis and proper professional help should be obtained to manage as well as to acquire better control of diabetes mellitus.
