Karydakis flap operation for chronic pilonidal sinus.

Objective: To present the personal experience of treating Chronic Pilonidal Sinus with Karydakis flap operation in terms of effectiveness, healing time, complication rate and recurrence after surgery. Design & Duration: Retrospective observational study from Jan. 2000 to Nov. 2005. Setting: Shifa International Hospital, Islamabad; a tertiary care teaching hospital. Patients: 30 cases of Pilonidal Sinus operated with the Karydakis flap technique. Methodology: Detailed data of all the patients was recorded. The patients were operated by the Karydakis technique, which entails an asymmetrical elliptical incision around the sinus and closure with a lateral scar away from the midline. Details of the surgery, its complications and the outcome were noted down. Results: There were 28 males and two females in this study (M: F ratio 14:1). Their mean age was 20.5 years (16- 38 years).The mean hospital stay was 3 days (Range 2-7 days). The mean healing period was 14 days (Range 10- 26 days). The mean period off work was 16 days (Range 10-34 days). There were five minor complications (16.65%) in this series (superficial sepsis in two, medialization of scar in one and partial wound breakdown in two cases). No recurrence was noted within the mean follow-up period of 18 months (range 6-24 months). Conclusion: Karydakis technique appears to be a simple procedure for the management of Pilonidal Sinus, associated with low morbidity and earlier recovery.
