Management of tonsillectomy hemorrhage by bipolar versus dissection method.

Objectives: To compare the dissection method of tonsillectomy with that of bipolar electrodissection method in controlling haemorrhage. Materials and Methods: This is a prospective case series study conducted at the ENT department of Khyber Teaching Hospital, Peshawar from Jan 2003 to Dec 2004. The results of the two groups were studied regarding time taken in the procedure, postoperative analgesia required, reactionary haemorrhage and secondary haemorrhage. Results: The average time was 15 minutes with bipolar diathermy as compared to 30 minutes on average for tonsillectomy by dissection snare method. Frequency of primary haemorrhage was the same in both methods; however, secondary haemorrhage was more in bipolar method (13.2% vs 4.16%). Conclusions: It is concluded that although less time is consumed when using bipolar diathermy to achieve haemostasis during tonsillectomy, however the ligation of bleeding point using silk suture was more effective in reducing secondary haemorrhage.
