Xanthogranulamatous Pyelonephritis.

A case of xanthogranulamatous pyelonephritis is presented which was clinically diagnosed as a renal tumor. Nephrectomy was carried out with removal of perinephric fat and suprarenal gland. Histopathological examination confirmed the diagnosis of xanthogranulamatous pyelonephritis.

Case Report: A 38 years old patient presented with complaints of highgrade fever with rigors and left flank pain lasting for six weeks. Abdominal examination revealed tenderness in the left flank. No abnormality was detected in other systems. Urine analysis showed microscopic pyuria. His renal functions were within normal limits. An abdominal ultrasound (US) revealed a 3.8 x 4.4 cms solid mass with cystic areas involving the middle pole of left kidney while contralateral kidney was normal. CT scan confirmed the findings of US. With provisional diagnosis of renal tumour, exploration was carried out by flank approach. There was a solid mass in the middle pole of left kidney with perinephric adhesions. Nephrectomy was carried out with removal of perinephric fat and suprarenal gland. Hilar lymph glands were not involved. Postoperative period was uneventful. Pyrexia settled down on third postoperative day. Histopathological examination of the specimen revealed features consistent with the diagnosis of Xanthogranulamatous pyelonephritis (XPN).
