Partogram curves in Primigravidae.

Objective: A pilot program to observe the different pattern of partogram curves and to evaluate the outcome of deliveries in primigravidae. Place and Duration of Study: This was a hospital-based survey and contained a series of consecutive patients presenting at Hayat Shaheed Teaching Hospital from May 1997 till May 1998. Subjects and Methods: A prospective analysis of labor curves of 444 consecutive cases of primigravidae was carried out. The labor progress was recorded graphically and partogram curves were categorized using the WHO partogram. The need for neonatal resuscitation was also correlated with the crossing of Alert and Action lines in the partogram. Results: 62.38% of the primigravid labors had a normal curve whereas 4.07% had a prolonged latent phase, 8.1 had secondary arrest and 24.7% showed primary dysfunctional labor, 11.06 % babies needed resuscitation, 9.4% of these were in labors in which the alert line had been crossed. Conclusion: Partogram can be used in tertiary care hospitals for optimum labor management as early deviations from normal could be detected easily. Increased need for neonatal resuscitation correlate with the crossing of alert line on partogram.
