Clinical Profile of Acute Rheumatic Fever in Pakistan - A prospective study.

Rheumatic Fever (RF) is the commonest cause of acquired heart disease among children and adolescents in the developing world. Clinical spectrum is reported to differ markedly in different parts of the world, being mild in the developed. versus severe is the developing world. The purpose of this study was to determine the clinical profile of ARF in First episode (FE, Group I) and recurrent group (RG, Group II). The period of study was from March 1991 to March 1994. Children who had ARF based on Mondified Jone`s criteria were included. 161 patients entered the study within the period of 2 years. Group I comprised of 57 patients and 104 patients had recurrent attach of ARF (Group II). Carditis was noted in 35 of 57 (61%) patients in group I compared to 100 of 104 (96%) in group II. Chorea was noted in 9 of 57 (16%) patients in group I and 4% of cases in Group II. Skin lesions in group I were in 6 of 57 (10.5%) patients. In group II, subcutaneous nodules were noted in 6% and 1% of patient had erythema marginatum.
