Management of High Fistula-in-Ano.

This is a prospective study comprising 20 cases of high (trans-sphincteric and supra-sphincteric) fistulae-inano managed by firm setoning. This study was carried out at Lahore General Hospital and Jinnah Hospital Lahore, from May 1993 to July 2000. Twenty patients were included in the study. Eighteen were male and two females. Diabetics and patients with inflammatory bowel disease were not included. Majority of the patients had past history of multiple surgeries for fistula in ano. Among these patients, sixteen healed completely (88%), two patients were lost from follow up and two showed no evidence of healing after surgery. We conclude that `Seton` insertion is not only a safe method of treatment for high trans-sphincteric and supra-sphincteric fistula but also helps to identify borderline high and low trans-sphincteric fistula.
