Sphincter saving anorectoplasty (SSARP): Our experience at the Department of Paediatric Surgery QAMC Bahawalpur.

Introduction: Anorectl malformation is common congenital anomaly with wide spectrum of defects. More common in males. High divided sigmoid defunct colostomy is usually performed as a fi rst stage surgery in newborn with high and intermediate anomaly. Posterior sagital anorectoplasty (PSARP) developed by De Vries and Penna is performed later on as defi nite procedure. Sphincter saving anorectoplasty is a new technique with modifi cation in Penna repair with much bett er results regarding continence. Objective: Th e objective of this study is to analyse the outcome of this modifi ed procedure in terms of faecal continence and related complications Study design: It is descriptive case series conducted at department of paediatric surgery Bahawal Victoria Hospital Bahawal Pur from January 2007 to December 2012. Results: A total no of 24 cases were included. Gender distribution 83% n=20 males and 17% n=4 females. Regarding continence 70.8% n =17 had excellent, 12.5% n=3 had good, 8.3% n=2 had fair and 8.3% n=2 had poor continence. Regarding complication, anal stenosis presenting with constipation noted in 4% n=1 , 4% n=1 had mucosal prolapse , 8% n =2 had facal soiling and 4% n =1 had urinary retention . Conclusion: Sphincter saving ano rectoplasty (SSARP)is a bett er alternative to original Penna procedure with the advantage of saving the external sphincter tissue and anatomical reconstruction of neo anus with satisfactory function and cosmetic outcome
