Does Alvarado score preclude a negative appendicectomy?.

Objectives: To determine the frequency of negative appendicectomies in cases of acute appendicitis diagnosed on the basis of Alvarado Scoring System. Sett ing and duration: Department of Surgery, Unit VI of Dow University of Health Sciences and Civil Hospital Karachi from February 2009 to August 2009. Study design: Descriptive case series Methodology: 77 patient who visited outpatient department and emergency department with complaint of right iliac fossa pain, having Alvarado score >7 were operated and appendix was sent for tissue diagnosis. Frequency of negative appendicectomy was then determined on the basis of histopathological fi ndings (i.e. normal vs. infl amed). Statistical analyses were performed on SPSS (version 17). Results: Alvarado Score 9 was encountered in 25 (32.5%) cases followed by score 8 in 23 (29.9%), score 7 in 18 (23.3%), and score 10 in 11 (14.3%) participants. Th e rate of negative appendicectomy was 16.9% (n=13). Conclusion: Th ere is a constant patt ern of negative appendicectomies using Alvarado Scoring system, consistent with international literature. Females in reproductive age group are more likely to have negative appendectomy with the use of this scoring.
