An immediate exploration of penetrating upper extremity injury for unexpected vascular trauma: a case of limb salvage.

A case of penetrating injury to arm in a twenty three years old male is reported. He got his right arm injured from a sharp object (a grinder blade) above the cubital fossa and presented with profuse bleeding. On clinical examination, brachial, radial and ulnar pulses were equally present but feeble bilaterally. Th e patient had no hard and soft signs of vascular injury. No neurological defi cit was found. Wound was explored under general anesthesia for repair of injured muscles but revealed brachial artery injury per operatively. Arterial circulation of right arm was restored with use of reverse saphenous vein interposition graft with signifi cant improvement in clinical outcome.
