Incidence of mammary fistula after incision and drainage of breast abscess and its management.

Introduction: Breast abscess is the most common emergency requiring surgical management in lactation mothers. Th e most frequently used treatment is incision and drainage which is sometimes followed by milk fi stula, a dreadful complication. Milk fi stula is a non healing persistent abnormal communication between the mammary duct and skin which only responds to surgical fi stulectomy in most cases. Non-surgical management of milk fi stula is a new modality. Objectives: To study the incidence of milk fi stula in all patients undergoing incision and drainage for breast abscess and to evaluate its response to anti-prolactin drug treatment. Study design: It is a prospective study of 60 patients conducted in the surgical department of Khyber Teaching Hospital, Peshawar over a period of 3 years. Results: 6.6%(n=4) patients developed milk fi stula, all of which were <30years of age. Bromocriptine showed a 100% result in all cases of milk fi stula, none of which required surgical intervention. Conclusions: Milk fi stula is relatively more common in our setup especially in young patients. Bromocriptine and stopping lactation is a safe and eff ective method of treatment in this condition.
