Comparison of in-hospital outcome of patients having first acute myocardial infarction with or without pre-infarct angina.

Objective: This study was conducted to compare in-hospital outcome of patients with first AMI in terms of mortality and cardiogenic shock with or without pre-infarct angina. Materials and Methods: This cohort study was conducted at the Punjab Institute of Cardiology, Lahore from July to Dec 2010. The study included 150 consecutive patients with acute myocardial infarction. They were distributed into two groups; one without preinfarct angina and second with preinfarct angina. The main outcome measures were in-hospital mortality and cardiogenic shock. The data was analyzed by using software SPSS. The numerical variables were presented as mean and standard deviation while qualitative variables were presented as frequency and percentage. The two groups were compared using RR (relative risk) and a p value of < 0.05 was considered significant. Results: Of 150 patients , 75 in each group. There were 70.7% male in Group 1 and 82.7% male in Group 2. The mean age was 54.28 ± 9.70 in Group 1 vs. 52.08 ± 10.18 in group 2. Risk factors for coronary artery disease were more common in Group 1. Patients in Group 1 had more complications as compared to Group 2. Cardiogenic shock was 18(24%) in Group 1 vs. 2(2.7%) in Group 2 with RR =9.00. In-hospital mortality was 17(22.7%) deaths in Group 1 vs. 2(2.7%) in Group 2 with RR (Relative risk) = 8.5 Conclusion: Presence of preinfarct angina in patients of AMI results in lower incidence of cardiogenic shock.
