Phacoemulsification Under Topical Anaesthesia and Superior Rectus Infiltration.

An analysis of 400 cases of phacoemulsification under topical anaesthesia and superior rectus infiltration is presented. All of the cases were operated on by anterior limbal corneal incision. In 340 eyes posterior chamber intraocular lens implantation was performed. Sixty eyes were operated on for extracapsular cataract extraction (ECCE) without intraocular lens implantation. In 260 eyes the corneal wound was closed with one stitch of 10-0 monofilament nylon. In the remaining 140 eyes the corneal wound was left unstitched and stromal hydration was performed at the wound edges. Much improved postoperative visual acuity with better colour perception was achieved in the immediate postoperative period compared to the conventional techniques of regional anaesthesia. A few postoperative complications, like flishmouthing, of corneal wound, corneal oedema and posterior capsule tear were noted.
