Treatment of Endophthalmitis with Intravitreal Injection of Antimicrobials.

Purpose: Aim of the study was to analyze the results for visual outcome, anatomical integrity and post-treatment complications. Methods: Seventy-two eyes clinically diagnosed as cases of endophthalmitis were treated with intravitreal injection of antimicrobials during the period of 3 years from Dec.`93 to Nov.`96. Cases of endophthalmitis after surgery, trauma, keratitis or endogenous infection were included. All the patients were given intravitreal injection of antimicrobials following vitreous aspiration or AC tap. PPV (pars plana vitrectomy) was performed when vitreous could not be aspirated. Postoperative follow-up ranged. from 6-48 months, the average being 25 months. Results: The commonest route of entry was noted to be an ocular operative procedure in 38 (53%) and accidental trauma in 21 (29%) cases. Cultures were positive in 53 (74%) of the cases, Staph aureus being the commonest pathogen isolated. Post-treatment 6/6 to 6/24 vision could be achieved in 21 cases (29%), while 28 (39%) had 6/36 to PL positive and 23 (32%) were PL negative. Anatomical integrity was preserved in 97.2% of cases. Conclusion: Intravitreai delivery of specific antibiotics (with or without pars plana vitrectomy) is the accepted norm in modern ophthalmological practice for treating endophthalmitis.
