Prophylactic Management of Retinal Detachment.

In this study we compared two treatment modalities for treating retinal detachment prophlylactically. We chose two types of retinal breaks and treated them with photocoagulation or cryotherapy, and cryotherapy along with 360-degree encirclement. We selected 30 patients (30 eyes), out of which twenty two (73%) were male and eight (27%) were female. We divided these patients into two different groups on the basis of associated clinical findings along with the retinal breaks. Group One: This group consisted of fifteen patients with no more than three retinal breaks, retinal breaks with little or no associated vitreous traction or vitreous haemorrhage. Treatment modalities used In this group were argon laser photocoagulation in twelve patients (80%) and cryotherapy in three patients (20%). Group Two: This group also consisted of fifteen patients. In this group patients with large retinal breaks, more than three breaks, and breaks associated with vitreous traction, vitreous haemorrhage, or contracted vitreous base were included and treated with cryotherapy along with 360-degree encirclement (explant). The patients were followed for 6 months. Majority of these patients, i.e. eleven patients in group one (73%) and ten patients in group two (67%) had a smooth postoperative clinical course. However, we did come across some complications as well. In Croup One postoperative complication rate was 26.6% (four patients). In Group Two peroperative complication occurred in one patient (6.6%) and the rate of postoperative complications was 33.3% (five patients).
