Treatment of Post-Herpetic Neuralgia with subcutaneous injections of a mixture of Xylocaine, Bupivacaine and Dexamethasone.

Fifty two patients (32 males and 20 females) of post-herpetic neuralgia not responding to commonly used medicinal treatment modalities were given subcutaneous injections of a mixture of 2% xylocaine (20ml), 0.5% bupivacaine (20ml), and 0.4% dexamethasone (2ml). The mixture was prepared in a 50 ml syringe at the time of injection and 4-5 ml of the solution was injected subcutaneously at various sites in the region of PHN (5-10 sites in each patient). Patients were followed up regularly at six weekly intervals. If pain was not substantially relieved with a single injection, further injections were given at following visits (maximum upto 3 injections). Ten (19.2%) patients obtained complete relief from pain after a single injection, another 21 (40.4%) patients after second injection and 11(21.2%) patients after the third injection. Ten (19.2%) patients were resistant to the treatment. The only side effect noted was transient mild giddiness in 4 patients shortly after the injections. There was no recurrence of the pain on follow up for three months.
