Acquired Adult Megacolon Secondary to Angiodysplasia.

A 72-year-old Saudi female developed marked megacolon with lack of colonic motility and functional obstruction. She underwent total resection of her colon. Grossly, the colon was markedly dilated. On opening, the colonic circumference was 25 cm. The mucosal surface had become remarkably smooth. Microscopically, the ganglion cells were morphologically normal. The smooth muscle layers were markedly thinned out. There was pronounced vascular ectasia and vascular proliferation involving all the layers of the colonic wall including serosa and even the mesenteric fat. The lymph nodes had undergone significant angiomatous transformation. A diagnosis of angiodysplasia was rendered. The pathogenesis of the vascular changes and its relationship with development of megacolon and motility problems is discussed.
