Role of Transhiatal Esophagectomy in Carcinoma of Esophagus.

Transhiatal esophagectomy has a definite role in carcinoma esophagus. This procedure was done on 50 patients from January 1991December 1993. Majority of the patients included had stage-3 carcinoma. Median age was 65 year. Diagnosis was based on careful history, contrast studies, endoscopy and biopsy. 80% of tumors were squamous cell carcinoma and 20% adenocarcinoma. 50% of all tumors were on middle third of esophagus. Complications associated with this procedure were, anastomotic leak 8%, recurrent laryngeal nerve prexia 10%, pneumothorax (bilateral 10%, unilateral 8%) and pleural effusion 4%. Mean survival rate was one year. This procedure had an acceptable overall survival rate of 20%.
