Nasopharyngeal Angiofibroma: Surgical Technique & Preoperative Embolisation.

Nasopharyngeal angiofibroma is a disease of adolescent males. It presents with nasal obstruction and epistaxis. We present a retrospective study of 20 cases treated over a period of five years, from May 1995 to May 2000, to analyse the advantages of preoperative embolisation and wide exposure provided by the extended lateral rhinotomy approach. First five cases were operated by transpalatal approach, without preoperative embolisation. The subsequent cases were approached by lateral rhinotomy, with or without lip splitting incision, within 24 hours of embolisation of the internal maxillary artery. Absence of embolisation is associated with substantial intraoperative blood loss, while in embolised cases the average blood loss is 700 ml. Transpalatal approach provides very limited exposure and is suitable only for tumours localised to the nasopharynx. Lateral rhinotomy incision is good for lesions extending into the paranasal sinuses. Extended lateral rhinotomy provides wide exposure for removing tumours extending into the infratemporal fossa and skull base.
