Euglycaemic Diabetic Ketoacidosis.

The term Euglycaemic diabetic ketoacidosis has been suggested for a syndrome of severe diabetic ketoacidosis with plasma bicarbonate of 10 mmol/L or less and a relatively normal blood sugar (16.7 mmol/L or 300 mg/dl or less) on admission to the hospital. In this report, we describe our experience of three such cases in Madinah AI-Munawarah, Saudi Arabia.

CASE REPORT: These 3 cases were seen in King Fahd Hospital (2 cases) and King Abdul Aziz Hospital (1 case) Madinah Al- Munawarah, Saudi Arabia. The diagnosis was based on clinical features with glycosuria and ketonuria with blood sugar > 1 m.mol alongwith arterial pH < 7.3 and bicarbonate < 15.0 mmol/L. Infection was ruled out by clinical features, chest X-ray, blood and urine cultures. All the patients were treated by Normal saline initially followed by 5% dextrose when the blood sugar was about 11.0 m.mol/L alongwith low dose regular insulin either intramuscularly or by intravenous infusion pump. All of them gradually recovered and were discharged on combination of regular and NPH insulin twice a day.
