Adequacy of Hemodialysis.

Objective: To determine the delivered dose of haemodialysis and factors responsible for inadequate dose of haemodialysis. Design: Single-centered prospective hospital based study. Place and Duration of Study: Haemodialysis unit of Shaikh Zayed Hospital, Lahore from March to May, 2000. Patients and Methods: Fifty-one patients on regular haemodialysis were included. Three blood samples were drawn to check adequacy of dialysis and recirculation. Adequacy was checked by Kt/V, urea reduction ratio (URR), time average urea clearance (TAC urea). Symptoms of uremia were asked and physical examination done. Nutritional status was checked with anthropometic measurements and laboratory datas (hemoglobin, albumin). Results: In the present study, total number of patients were 51. Twenty-four patients were male and twenty seven female. Mean age was 43 years. Main cause of end stage renal disease was chronic glomerulonephritis followed by diabetic and hypertensive nephropathy. Majority of patients 35 (61%) were getting inadequate dialysis i.e. Kt/V less than 1.4. Re-circulation was less than 10% in all patients. Patients with inadequate dialysis were having low serum albumin and hemoglobin. Conclusion: Majority of patients were getting inadequate dialysis. Factors responsible for inadequate dialysis were financial and social problems, inadequate haemodialysis time, non-availability of large surface area dialyser.
