Management of Unstable Lower Thoracic and Lumbar Spine with Transpedicular Fixation.

The goal of treatment of the unstable spine is the restoration of the normal, pain-free, static, dynamic and protective functions of the spine. Stable healing of the spine in its anatomic form is a pre-requisite to achieve this goal. The common unstable conditions presenting to us include spinal injury and Caries spine while a few cases of spondylolesthesis are also treated. Pedicle screw placement provides three column fixation to the spine as the screws go well into the vertebral body, thus incorporating the vertebral body in the posterior element construct. Our series include 115 patients among which 90 patients are of spinal injury and 25 cases of Caries spine. All these patients underwent Transpedicular fixation with DC plates attached with pedicle screws on both sides. The results of our study strongly suggest that with locally made, unconventional pedicle screw plate system, uniformly good results could be achieved without any significant complication rate.
