Tuberculous Splenic Abscess.

Splenic abscess is rare being reported in 1 per 10, 000 discharges. Tuberculosis splenic abscess is even rarer. A case of splenic abscess is being reported where tuberculous etiology surfaced at laparotomy for splenectomy.

Case Report: A 24 years old unmarried lady with splenic abscess was referred to us on 2nd July 1991 From medical department after full work up. She had fever, anorexia and weight loss for 2 months. Examination revealed tender splenomegaly. Her leukocyte count was 10,400/mm, and ESR 60mm. At the end of 1 st hour. X-ray of chest was reported normal so were her liver function tests, serology for hydatid disease and hematology screen. Ultrasonography showed an 8 x 6 cm fluid containing lesion in an enlarged spleen. A laparotomy was performed on 18th July 1991 revealing peritoneal cavity studded with tubercles. An uneventful splenectomy was performed. Histopathology revealed it to be a case of tuberculous spleen abscess. X-ray chest repeated on 25th July 1991 at the time of discharge showed miliary tuberclous lesions in both the lung fields. The patient was put on anti tuberculous treatment for 1 year.
